The following informatin must accompany all applications.  If any required information is not submitted with application, application will not be processed.

1. $50.00 Application fee per single adult over the age of 18.   
2. Proof of employment (Copies of 2 month's pay-stubs or letter of new employment showing start date and amount of wages.)
3. Copy of current photo ID.
