The following information must accompany all applications.  If any required information is not submitted with application, application will not be processed.

1.  Application fee is $50 per applicant.  This is a non-refundable fee and must be paid in cash or certified funds.  No personal checks.

2.  Proof of employment (Copies of 2 month’s pay-stubs or letter of new employment showing start date and amount of wages.)

3.  Copy of current drivers license or official state photo ID.
